If you are a Nursing Home, Retirement Home, Private Home or Detention center, use the Requisition form below.

Authorization Form

If you are a Nursing Home, Retirement Home, Private Facility or Detention center, and are not part of our Schedule A but would like to utilize our Mobile X-Ray or Ultrasound service, use the form below.

Please complete the following 3 steps before faxing to MDI:

How to Fill the Authorization/Approval Form:

1. The physician/NP must fill out the Ministry of Health Mobile X-Ray & Ultrasound Authorization form and fax it to the MOH at their Kingston Office for approval signature

Ministry of Health will respond to your request within 24 hours.

2. Once the Form has been approved and signed by the ministry they will fax it back to you.

3. You can then fax the MOH approved form to us at MD Imaging and we will schedule a technologist for your resident.
